Passenger Request Form

AirLink is committed to the safe and efficient transport of patients and crew members. A passenger request form is now required for family members accompanying a patient during a flight, when an EMS resource flies with a patient from a scene and when a third rider is scheduled to fly with the AirLink crew.

The request form captures emergency contact information and provides the rider with an understanding of what is expected for the flight.

Completed forms must be faxed to AirLink's Dispatch Center at 541-706-5809.
