For New Patients
When you're new to cancer treatment, you'll want — and need — clear, simple and complete information about how the process works, what to expect, and what can be done to manage symptoms that occur.
Your oncology nurses (cancer nurses), radiation therapists (therapists), radiation oncologists (radiation therapy doctors), and medical oncologists (chemotherapy/hematology doctors) will be a great source of information.
Prior to your first visit, you may be instructed to download, print out, and complete the applicable forms and bring them with you to your visit:
- Patient Questionnaire for New Patients (all cancers except prostate) — For all patients to complete
- Patient Questionnaire for Prostate Cancer Patients Only
- General Patient Information — For Radiation Oncology patients
- Genetic risk evaluation and testing form
